Membership Suspension Request
As stated in your membership agreement, you have the option to put your weekly membership on hold for any reason.
This form will serve as your 7-day written suspension notice as required by your membership agreement.
If you request a suspension date that is within 7 days of the form submission, your membership will instead be suspended 7 days from the date of the form submission.
Membership suspension requests must be for a minimum of 2 weeks, in 1 week increments, up to a maximum of 8 weeks (i.e. 2, 3, 4, 5, 6, 7 or 8 weeks).
Upon expiration of the suspension period that you select below, your account will automatically reactivate and regular membership payments will resume.
If you choose to cancel your membership during the suspension period, the standard 7-day written cancellation notice requirement stated in your membership agreement is applicable.
If you choose to reactivate your membership after it is cancelled, membership prices in effect at the time of reactivation will be applicable (membership prices are subject to change).
